Gift Exchange

Gift Exchange is built on an Assets Based Community Development model which is based around the idea that health is influenced by social determinants and that by re-introducing a person into their community and activities they enjoy, we can improve their health and wellbeing.

Living Well, Taking Control: Pre Diabetes

With partners, we have developed a unique approach to helping people at risk to avoid developing Type 2 Diabetes. We call it Living Well, Taking Control: Pre-diabetes and following a trial of the programme with patients in Birmingham on behalf of the South Central Clinical Commissioning Group (CCG), the programme has now become Healthier You: the NHS Diabetes Prevention programme. We are now one of four providers of this programme across the UK.

Living Well, Taking Control: Type 2 Diabetes

Health Exchange’s Type 2 diabetes programme is designed to be educational, but in a relaxed environment. Participants discuss various aspects of their lives that could be adjusted to bring about better health.
The group meets for four weekly sessions, which cover a range of topics related to both managing and de-escalating their condition.

Social Prescribing Service

This service aims to to encourage people over 16 years of age to engage with non-medical activities that can improve their health and wellbeing. We maintain a comprehensive database, which residents can use to search for activities near them, including sports and leisure groups, classes and clubs.

Feel-Good Mental Wellbeing Support

Our Feel Good Psychological Wellbeing Service adopts a ‘feel good model’ to promote a sense of wellbeing at initial contact and throughout the case formulation, support and exit strategies. We offer counselling and various other modes of support. 

Wellbeing Coordinator

Working with over 50s and those who care for another person, our Wellbeing Coordinator service engages clients in positive conversations, empowering them to improve their physical, mental and social wellbeing.


WorkStyle is Health Exchange’s employee-wellbeing offer to businesses of all shapes and sizes. The service promotes health in the workplace. Our portfolio of services includes Health Checks, Pedometer Activity, Stress Management, Sports Days, Smoking Cessation and much more.
