Job Title: Black Country Coordinator
Accountable to: Chief Operations Officer
Responsible to: Operations Manager
Location: Black Country Hub
Salary: £21,000 per annum
Hours: Full Time – 37.5 hours
Closing date: Wednesday 7th March
Throughout the West Midlands Health Exchange CIC deliver Health and Wellbeing services to support individuals who require support in a lifestyle change, whether that’s healthy eating, reducing alcohol consumption, becoming more social active, understanding mental health or support for carers and cared for.
Health Exchange CIC has successfully been delivering Stop Smoking services within The Black Country for the past 5 years. We are looking for an experienced people manager with service coordination skills to help us continue to thrive within the area.
Service elements:
Stop Smoking across the Black Country – development of those services to ensure that Payment by Results Contracts are on target. Linking into Pharmacies, GP Practices, Dental and Opticians and community groups and organisations.
Use of Lifestyle Coaches to deliver Gold standard services across all Payment by Results Contracts in the Black Country
Developing relationships with partners to enable the growth of the Stop Smoking service to support the Payment by Results Model (PBR)
Working and communicating with the Operations Manager to ensure that the trajectory of the Payment by Results contracts is meeting the successful trajectory and implementing any agreed measures to ensure successful trajectory if there is a short fall
Supporting the growth of Health Exchange CIC within The Black Country to offer greater reach into new service delivery areas
Build key relationships with stakeholders throughout the area who we can partner with to deliver services, expand service provision and work with to provide the best possible outcomes for local residents within The Black Country
Overview of Role:
The Black Country Co-ordinator will oversee the Payment by Results (PBR) programmes and its successful delivery. This will encompass service delivery, service development and area development to support Health Exchanges’ vision to improve the health and wellbeing of residents of The Black Country. The role will require leadership and the management ability to direct the Payment by Results programmes across the delivery sites in Sandwell, Walsall, Dudley and Wolverhampton
Key elements of the role:
1) To oversee the successful delivery of the Payment by Results Programmes in Sandwell, Walsall, Wolverhampton and Dudley
2) To provide support to, and work collaboratively with, the delivery teams across all sites and to organise shared learning opportunities
3) To work with the GP Practices, Dental and Opticians and community groups and organisations to ensure positive interest and engagement in the Lifestyle programmes supporting the Payment by Results (PBR) Contracts
4) To engage with evaluation and ensure effective communications and understanding with the delivery teams supporting the development of the services to support successful Payment by Results (PBR) delivery
5) To drive forward the programme delivery and work with Lifestyle Coaches to adapt the model where necessary to achieve best results
6) To attend relevant conferences and meetings for raising awareness of the delivery model
7) To build key relationships with partners and stakeholders
Key outcomes for the role:
1) Maintain quality of delivery standards across all sites
2) Achieving required and timely data collection to inform Payment by Results (PBR) trajectory, reports and Commissioner requirements
3) Developing strong Relationships with partners within the Black Country area
4) Developing strong relationships with community groups and leaders within the Black Country
5) Achieving high levels of activity and sustained attendance through the programme delivery
Person Specification
Knowledge, Skills and Experience
Skills, qualifications and experience necessary:
- Experience of working in health lifestyle interventions (Stop Smoking services/Health Checks/Other Lifestyle services)
- Relevant qualification within health improvement and social care, lifestyle services or sport – minimum level 3
- Ability to work with people at all levels
- Strong background in programme management and service delivery
- Direct and indirect management experience – ability to lead teams
- Excellent verbal and written communication skill i.e. responding effectively both in person, in writing and over the telephone, excellent presentation skills
- Car driver with clean license, with access to a car for work purposes
And preferably:
- Experience of working in the non-for-profit sector and commercial organisations
- Good Understanding of Local Areas across the Black Country – in the first instance Sandwell, Walsall, Wolverhampton, Dudley
