The purpose of this role is to facilitate clients within the community to improve their level of wellbeing by engaging in community assets. They will provide a seamless referral system for several GPs and other Healthcare professionals in the Primary Care Network (PCN) and ease access and engagement.
The role will require supporting clients to access a range of health, social care, welfare advice and other support services. It will require proactive engagement with healthcare professionals to stimulate referrals into the programme. Clients will be supported on an individual basis to set goals and priorities to improve their health and wellbeing.
Fundamentally the Link worker will be the point of contact for the Primary Care Network and facilitate the smooth implementation and adoption of social prescribing throughout the network. Engagement at practice and PCN level is essential, as is relationship building to embed a successful and productive service.
Integral to this role is building up the directory of services for north Warwickshire and coordinating the support options available to patients within this area. Developing relationships within local voluntary, community and social enterprise organisations will be key to this success of Social Prescribing.
