How does it work?
Our Health Trainers offer a personal one-to-one. You will work with them through 6-8 sessions in which you will meet and discuss your progress. You will be supported to make small but significant changes. By working together, you will be enabled to examine and overcome barriers, giving you the tools to maintain a healthy lifestyle. They can support, encourage and motivate you to make changes by working with you to create a personal health plan. We offer local services and offer appointments at a time that fits your needs.
Who is it for?
Birmingham: Anyone over the age of 16 can access the service.
Dudley: Anyone over the age of 18 can access the service.
How can I apply?
For both Dudley and Birmingham referrals, download and print a version of our Health Trainer Referral Form and post it to Health Exchange, 27 Moseley Road, Avoca Court, Birmingham B12 0HJ or you can follow the online application form at the top of the page. Alternatively, if you have a Birmingham based GP you can request for them to refer you to the program.
Case Study

Gail was referred to the Health Trainer Service in January 2009 by her GP. She was desperately unhappy with her weight, at 23st.8lbs and a BMI of 53.7.She also suffered from type 2 Diabetes, Asthma and Depression. Gail had booked a family holiday for August and was terrified she would not be able to fit into the plane seat . This, and support from the Health Trainer, provided her with motivation to lose weight and to save embarrassment. Through an assessment with her Health Trainer, they agreed a goal setting for weight reduction, exercise and nutrition. After completing the programme, Gail was able to fit in the seat without embarrassment, after losing 4 st.4lbs. and commented, “I am finally beginning to like myself”. Gail is determined to continue with her healthy eating and exercise regime. “I now know I can achieve if I just stick at it and don’t beat myself up if I have set backs!” She tributes her success to the Health Exchange and her Health Trainer and would recommend the service to her friends and family.
